Job Overview
We are seeking a dedicated Care Coordinator to oversee and manage the delivery of high-quality care services. In this role, you will ensure seamless coordination between clients, caregivers, and agency staff while maintaining excellent communication and operational efficiency. Your ability to organize, problem-solve, and provide exceptional service will be key to ensuring client satisfaction and caregiver support.

Key Responsibilities

Care Management & Coordination

  • Match caregivers with clients based on needs, skills, and availability.
  • Monitor and ensure the continuity of care by proactively addressing scheduling changes.
  • Conduct regular check-ins with clients and caregivers to assess satisfaction and address concerns.

Problem-Solving & Crisis Management

  • Address last-minute cancellations, scheduling conflicts, or urgent client needs efficiently.
  • Identify potential gaps in coverage and implement proactive solutions.
  • Collaborate with caregivers and clients to resolve concerns in a timely and professional manner.

Communication & Relationship Building

  • Act as a primary point of contact for caregivers, clients, and agency staff.
  • Provide timely updates and clear communication to all stakeholders.
  • Foster strong relationships with clients to ensure a high standard of care.

Administrative Support & Compliance

  • Maintain accurate records of client care plans, schedules, and shift changes.
  • Ensure compliance with company policies, procedures, and regulatory requirements.
  • Document caregiver availability and maintain up-to-date records.

Technology Utilization

  • Utilize scheduling software and productivity tools for efficient coordination.
  • Track real-time updates and staffing changes to optimize workflow.

High-Volume Call Handling

  • Handle a high volume of inbound and outbound calls daily.
  • Expect to manage at least 100 calls per day.
  • Ensure professional and efficient communication with caregivers, clients, and agency staff.

Qualifications & Requirements

Experience & Skills:
✅ Proven experience in care coordination, case management, scheduling, or a similar role. (Required)
✅ Background in healthcare, home care, or caregiving is a plus.
✅ Strong problem-solving skills and the ability to manage multiple priorities.
✅ Excellent communication skills (written & verbal) in English.
✅ High level of organizational and time-management abilities.
✅ Proficiency in Google Workspace (G Suite) & Microsoft Office.

Technical & Work Setup:
✅ High-speed internet connection (at least 25 Mbps).
✅ PC/Laptop: Minimum 8GB RAM, i5 processor or equivalent.
✅ Noise-canceling headset (preferred).
✅ Quiet & professional home office setup.
